These Terms and Conditions govern your use of the PlatinumMeds mobile application operated by Justin Pham LLC. By downloading or using PlatinumMeds you agree to these terms.
Acceptance of Terms
By creating an account and using PlatinumMeds you confirm that you are at least 17 years of age or have parental consent and that you agree to be bound by these Terms and Conditions.
Description of Service
PlatinumMeds is a kidney medicine management app designed to help dialysis patients, CKD patients, transplant patients, and their caregivers manage daily medicines, track health data, and improve medication adherence.
Medical Disclaimer
PlatinumMeds is not a medical device. It is not intended to diagnose, treat, cure, or prevent any disease or medical condition. PlatinumMeds is not a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of your nephrologist or qualified healthcare provider regarding your medicines and treatment plan. Never disregard professional medical advice or delay seeking it because of something you read or saw in PlatinumMeds.
Subscription Terms
PlatinumMeds offers three tiers. Free tier at no cost with limited features. Plus tier at $2.99 per month or $29.99 per year. Platinum tier at $4.99 per month or $59.99 per year. Both paid tiers include a 7-day free trial for new subscribers. Subscriptions automatically renew unless cancelled at least 24 hours before the end of the current billing period. You can manage and cancel your subscription through your App Store or Google Play account settings. Deleting the app does not cancel your subscription. Refunds are handled according to Apple App Store and Google Play Store policies.
User Responsibilities
You are responsible for maintaining the confidentiality of your account credentials. You are responsible for all activity that occurs under your account. You agree to provide accurate information when creating your account and entering health data. You agree not to use PlatinumMeds for any unlawful purpose.
Caregiver Access
When you invite a caregiver you grant them access to view your health data within PlatinumMeds based on your subscription tier. You can remove caregiver access at any time through your app settings. You are responsible for who you invite as a caregiver.
Intellectual Property
PlatinumMeds and all its content, features, and functionality are owned by Justin Pham LLC and are protected by intellectual property laws. You may not copy, modify, distribute, or create derivative works based on PlatinumMeds without our express written permission.
Limitation of Liability
Justin Pham LLC shall not be liable for any indirect, incidental, special, or consequential damages arising from your use of PlatinumMeds. Our total liability to you shall not exceed the amount you paid for your subscription in the past 12 months.
Termination
We reserve the right to suspend or terminate your account if you violate these Terms and Conditions. You may delete your account at any time through the app Settings.
Changes to Terms
We may update these Terms and Conditions from time to time. Continued use of PlatinumMeds after changes constitutes acceptance of the new terms.
Governing Law
These Terms and Conditions are governed by the laws of the State of California.
